1954. U.S. Marshalls Chuck Aule en Teddy Daniels arriveren op Shutter Island, een eiland voor de kust van Massachusetts. Het duo moet er in de psychiatrische instelling voor geestesgestoorde criminelen de verdwijning van moordenares Rachel Solando onderzoeken. Teddy denkt dat Solando zich nog steeds op het eiland bevindt en wanneer de dokters van het ziekenhuis weigeren mee te werken, vermoedt hij dat er meer aan de hand is.
